How we work with you

Through the Integrated Care programme as part of our Transformation Package (more information here), My Home Life brings together care home managers and representatives from the wider health, social care and housing system. In these sessions, My Home Life acts as a broker to help establish better partnership working and identify shared local issues to work on together. You can read about an example of one of these meetings on the Buckinghamshire County Council website.
