Bucks integrated care plan developed at Care Home Summit

Congratulations to Buckinghamshire on creating their action plan for supporting better integrated care between care homes and the wider health and social care sector.

The Plan was created at a Care Home Summit, held at the Gateway in Aylesbury on 6th November 2014. The summit was attended by representatives from local care homes, GPs, commissioners and representatives from local hospitals, and many others.

The Summit follows on from a strategic commissioning meeting with members of the CCGs and commissioners from Buckinghamshire County Council (Bucks CC).
